Why healthy habits matter

Many adults with disabilities want better health routines but feel overwhelmed, confused, or unsure where to start. Traditional services often focus only on appointments and emergencies, leaving daily habits unaddressed. That gap creates fragility — small health issues become bigger ones because no one is coaching the everyday routines that prevent them.

When coaching is consistent, health becomes a set of daily habits — not something you only think about during a crisis.

  • More predictable days and better energy
  • Stronger independence around food, hygiene, and personal routines
  • Better communication with doctors and support teams
  • Long-term stability that reduces emergency situations
  • Safer decision making in the home and community

What bttr Health is

bttr Health is a coaching track focused on simple, realistic wellness routines. It is not medical care, therapy, or clinical treatment. It is hands-on daily support for the habits that improve quality of life and build lasting independence.

Coaching is delivered one to one through ILS, as part of a personalized SDP spending plan, or as a blended routine track depending on your Individual Program Plan goals.

What a typical week looks like

Healthy routines grow with consistency and repetition. Coaching follows a predictable structure so participants know what to expect and can build real momentum over time.

1

Check in and review

Start with a brief check-in about the week. What was hard? What worked? Any health appointments or situations to address? This shapes the session.

2

Practice the routine

Work through the day's focus area — hygiene steps, a simple meal, a cleaning task, or preparation for an upcoming appointment. Step by step, with the right level of support.

3

Community or movement activity

Many sessions include a brief community activity — a walk, a grocery run, or a wellness-related outing that connects to IPP goals in a real-world setting.

4

Reflect and document

End with a short reflection on what was practiced and what to keep building. The coach documents progress toward IPP health goals same day.

Health safety and professional boundaries

Safety is a core part of bttr Health. Coaching does not replace medical professionals — it supports the daily habits and communication skills that reduce risk and improve stability between appointments.

Safety focus areas

Coaching that keeps participants safe and connected to the right support

Recognizing unsafe home conditions and knowing when to report them
Supporting preventive habits like handwashing, cleaning, and food safety
