Consent to Receive Services
Effective Date: January 1, 2025
At bttr., informed consent is the foundation of ethical, respectful, person-centered service delivery. Before services begin, every participant or authorized representative must understand and voluntarily agree to receive support through bttr.
What Is Informed Consent
Informed consent means the participant or authorized representative understands who bttr. is, what services are offered, how they are funded, how participation works, how personal information is used and protected, and agrees voluntarily to begin services. Consent must be free of pressure, confusion, or fear. Participants can ask questions, request accommodations, or withdraw consent at any time.
Who May Provide Consent
- The participant, if legally able
- Parent or guardian, if the participant is a minor
- Court-appointed conservator or authorized representative
- Designated support person with legal authority
Participants may involve family, advocates, or Regional Center staff when reviewing the consent process.
Services Covered
- Independent Living Services (ILS): One-on-one coaching on skills like budgeting, cooking, communication, health, and advocacy.
- Supported Living Services (SLS): In-home support focusing on routines, safety, housing, and autonomy, including 24/7 support if authorized.
- Community Integration Training Program (CITP): Small group coaching for social connection, job readiness, public safety, and independence.
Services are delivered under an approved IPP (Individual Program Plan) or SDP (Self-Determination Program) ISP (Individual Service Plan) and POS (Purchase of Services), with no out-of-pocket costs to participants.
Requirements to Begin Services
- Regional Center or SDP authorization
- Completed intake assessment
- Reviewed and agreed-upon initial ISP
- Residence in an approved service area
- Willingness to engage respectfully in services
Withdrawing or Ending Services
Participants can stop services at any time. We ask for 30 days' written notice for smooth transition. Services may end if consent is withdrawn, authorization ends, relocation outside the service area, goals are completed, non-participation occurs, or a serious safety or conduct violation arises. We commit to respectful, collaborative transitions.
Notice of Withdrawal
To end services, provide written notice to:
- bttr.: email info@bttrliving.com or notify a coach
- Regional Center Service Coordinator or FMS provider if under SDP
Submit at least 30 days in advance unless there’s an emergency or agreement to end sooner. For serious violations, bttr. reserves the right to end services immediately with written notice to all parties.
Consent to Communication
By consenting to services, participants agree to allow bttr. to communicate by phone, email, text, or in person, coordinate with Regional Center staff, maintain secure records, share required documentation, and request updated consent if services change. Participant information is never sold or shared outside authorized service use.
