Consent to Receive Services

A clear explanation of your rights, responsibilities, and choices before starting services with bttr. Informed consent ensures you understand our supports, how they work, and your freedom to ask questions or withdraw at any time.

What is informed consent

Informed consent means the participant or authorized representative understands who bttr is, what services are offered, how they are funded, how participation works, and how personal information is protected — and then agrees voluntarily to begin supports.

Consent must be free of pressure, confusion, or fear. Participants may ask questions, request accommodations, or withdraw consent at any time without it affecting how they are treated.

Voluntary

No one can be pressured or required to begin services. Consent is always a free choice.

Informed

You receive clear information about what services involve before agreeing to anything.

Reversible

You may withdraw consent at any time and services will be ended respectfully.

Protected

Your personal information is never shared outside authorized service delivery.

Who may provide consent

The following individuals may provide consent to receive bttr services. Participants are encouraged to include trusted family or advocates in discussions even when they are not the primary consent provider.

  • The participant, if legally able to consent on their own behalf
  • A parent or legal guardian, if the participant is a minor
  • A conservator or legally authorized representative
  • A designated support person with documented legal authority

Services covered by this consent

This consent applies to all services bttr provides. Services are delivered under an approved IPP and purchase of service authorization through a Regional Center or the Self Determination Program. There are no out-of-pocket costs to participants for any authorized service.

Independent Living Services

One to one coaching in skills like budgeting, cooking, communication, health, and self-advocacy. Delivered in the home and community through Regional Center ILS authorization or SDP.

Coordinated Family Support

Support for adults who live at home with family. Focuses on coordinating services, building routines, and reducing household stress. Available through RCEB and GGRC.

Tailored Day Services

Skill building, community access, and structured activities outside the home. Available through SDP statewide, billed in four-hour units.

Supported Living Services and In-Home Respite

SLS and IHR are available through the Self Determination Program only, statewide. SLS supports adults living independently. IHR provides scheduled in-home relief for families.

Requirements to begin services

  • Active Regional Center authorization or SDP spending plan approval
  • Completed bttr intake assessment and goal review
  • Reviewed and agreed upon initial Individual Service Plan or IPP goals
  • Residence within an approved bttr service area or SDP statewide eligibility
  • Willingness to participate respectfully in coaching and services

Withdrawing or ending services

Participants may stop services at any time. We request thirty days written notice to support a smooth and respectful transition. Services may also end for the following reasons.

Transitions are always handled respectfully and collaboratively

Consent is withdrawn by the participant or authorized representative. Regional Center or SDP authorization expires and is not renewed. The participant relocates outside the approved service area. All IPP goals are completed and no new goals are identified. Non-participation continues despite repeated attempts to engage. A serious safety or conduct issue arises that cannot be resolved.

Notice of withdrawal

To end services, participants may notify bttr by emailing info@bttrliving.com, contacting their coach directly, or notifying their Regional Center service coordinator or FMS provider under SDP.

Please submit notice at least thirty days in advance whenever possible. For serious safety violations, bttr reserves the right to end services immediately with written notice provided to all relevant parties.

Consent to communication

By consenting to receive services, participants allow bttr to communicate by phone, email, text, and in person as needed to deliver services effectively. This includes coordinating with Regional Center staff or SDP teams, maintaining secure service records, sharing required documentation with authorized parties, and requesting updated consent if services change significantly.

bttr never sells, shares, or uses participant information for any purpose outside authorized service delivery.

Participant rights

Every person receiving bttr services holds these rights at all times. These rights cannot be waived, restricted, or taken away.

Be treated with respect, dignity, and cultural sensitivity in every interaction
Ask questions at any time and receive clear, honest answers
Decline, pause, or modify services without retaliation or loss of support
Participate fully in planning, goal setting, and all service decisions
Access personal records and request corrections to any information
File a complaint or grievance at any time without fear
Choose who participates in service planning discussions
Request materials in preferred languages or accessible formats
