We accept your interest or referral and schedule a welcome call to learn goals, routines, and preferences. We confirm eligibility and hours with your Service Coordinator.
Assessment and match
A bttr assessor meets with you to understand strengths and support needs. We match you with the right staff and set a start date and communication plan.
Plan your path
We build your plan with clear goals, activities, and outcomes. You approve the plan with your family and your Service Coordinator.
Start services
Sessions use real tasks and real practice at home and in your community. We log notes after every visit and track progress.
Review and grow
We review progress monthly and quarterly. Goals update when skills improve or needs change. Families and case managers get concise summaries and next steps.
Long term support
We celebrate milestones and plan what is next. You can continue services, adjust hours, or transition support based on your goals.
First 30 days

Week 1
Welcome call, intake, consent, schedule setup, safety review, and communication preferences. Home and community orientation.
Week 2
Assessment visits and baseline checks across LIFE, MONEY, TECH, HEALTH, EXPRESS, and CONNECT. Draft goals and activities.
Week 3
Plan review with you and your circle. Confirm schedule, locations, and priority tasks. Staff match confirmed.
Week 4
Services start with measurable actions and simple trackers. First progress summary shared with family and your Service Coordinator.

What is not included
Emergency medical transport or emergency on demand response. Staff follow safety plans and contact emergency services when needed. We plan non urgent transport and backups.
Communication standard
We confirm schedules in advance. We send monthly summaries to families and case managers and reply within one business day.
