In Home Respite
A trusted support professional at home, so the family gets a planned break.
Behavioral Respite
Respite with support professionals who follow the team's existing strategies.
Independent Living Skills
Everyday skills learned on real tasks, at home and in the community.
Supported Living Services
Steady support in the participant's own home, week after week.
Personal Attendant Services
Hands on help with daily routines for adults who live with family.
Tailored Day Services
A weekday schedule built around the participant's interests, in the community.
Community Connections and Recreation Support
Social and recreational activities the participant chooses, with a coach alongside.
Adaptive Fitness Coaching
One to one coaching built around movement the participant enjoys.

Recommended rates for the Self Determination Program. They may be adjusted based on individual needs and approval by the Independent Facilitator and FMS. bttr. bills only what is approved in the spending plan.
